Windows BMP to Hires BIN

I'm wondering if there is any way to take a 280 by 192 pixel Windows
monochrome bitmap file and convert it to an Apple II Hi-Res graphics
binary file. If there isn't already a program to do this, it would
probably be something easy like stripping the header data and reorganizing
the data.
