In recent enough System Software (3.1 and later, I think). Once you go
back far enough, TOOL.SETUP is a monolithic file that contains all
patches, with appropriate ones applied to the system according to the
detected ROM version. There are no TSn files.
When they added the TS1 and TS2 files, TOOL.SETUP became a simple loader
for the other files. I'm pretty sure it didn't include any patches
itself at this time. In a later system version, common patches for ROM
1 and 3 were moved into TOOL.SETUP so that disk space could be saved (by
avoiding duplication in TS2 and TS3).
(I could confirm all this by reading through the release notes for
Systems 4 through 6, but I want to go to bed.)

As I said in my earlier posting, TS1 is the patch file for ROM 00, not
ROM 01. TS2 is the patch file for ROM 01. TS1 is completely ignored by
System 4.0 or later - you can't even boot GS/OS (System 4.0 or later) on
a ROM 00, so the TS1 file would never be used.
The only way you could end up with a TS1 file on a System 4.0 boot disk
is if you put it there yourself (or your source disk had it there due to
being a mixture of system versions). Official System 4.0 only has
TOOL.SETUP and TS2.
