Another possibility is a missing SOS boot block (block 1), as discussed earlier.
Well as Jerry pointed out a few posts back, the Block 1 thing turned out to be false. At least on the Apple /// that I have, the disk "Apple /// Business Basic," which has zeroes in Block 1 and the boot stuff in Block 0, works fine.
In the case of my Apple ///, the problem was that the drive mechanism was faulty, which luckily I was able to fix.
That said, I am still having issues with "SYSTEM FAILURE: $06", but Apple's Tech Notes say that: "Copying certain protected diskettes will cause an error $06 when the copy is booted. This is a normal effect of the protection scheme."
And I'm definitely not using original disks, since I did not win that auction earlier this year which had like, 1/4th the entire Apple /// library ... in original boxes... it was awesome and it made me want to cry. Software these days does not come in such sweet boxes. SOFTWARE USED TO COME WITH MANUALS! Big, thick, nice ones!!
